"Cómo estas" means "How are you?"
So your reply should be Muy bien(very well) / Bien(Well) / mas o menos(so so) / Mal(bad) / Muy Mal(Very bad)

"So and so" could also be translated as "sin más", you usually express this one a bit serious, after a couple couple of seconds, as if you were thinking for a suitable answer to the question.

I think that "so and so" may also be translated to "así así"

so and so means

1.	someone or something not definitely named: to gossip about so-and-so.

2.	a *; son of a * (used as a euphemism): Tell the old so-and-so to mind his own business.

así así,= so-so

Para "so and so" también puede decirse "regular".

"Sin más" here means "I am well, but, could be better".

Ana: ¿Qué tal estás?
Luís: Más o menos..

Honestly, if someone answered me that way, I would say "¿Qué?". "Más o menos" is translated as "more or less", isn't it?

Ana: How are you?
Luís: I am more or less..

..does that make sense? Maybe it does, but, I would say in Spanish it doesn't.
