Yes, my eyes were blue as a teenager. As "old age" set in, they began to change to their current color of green. I am not sure how it can be possible but it happened to me so I know it CAN happen.
Eyes can change "their" color not "its" color. "It's" is the contraction for "it is" so if "it is" will work in the sentence you are using, you can use it's.
