There isn't much difference between "como esta" or "como esta usted".Usted is used as affirmative pronoun.

And when it comes to other question: "como es" is for physical description such as tall,short,blond,skinny,etc.

So to ask "how are you" always one needs to say "como esta" in spanish.

Well most of the time with spanish speakers "como esta usted" is used very seldomly Most of the time when your speaking to a fluent spanish speaker you would ask "como esta?"Thats it, I wouldn't recommend worring about the usted. Its usually used in a very formal way like a form of respect, still not used very often now a days. To me personally it sounds a little funny. Now if someone asks you "hola como esta" then you would reply "muy bien y usted" to me its used better that way and its used way more often with fluent spanish speakers!
Its really optional how you want to use it and how you want it to sound!

Como esta ud, is a respectful version (Used to speak to teachers, parents, adults, etc...) of como estas. Como estas means how are you.

You is pronounced differently.

Ud = Respectful version

Tu = Slang, relaxed version ~ Used for friends.

Well, I don't mean to be too specific, but "yo" is already implied in "estoy" because it is an irregular pronoun. Adding the "o" to a verb in spanish means that the narrator is taking the action. But, estoy is an irregular. It confuses alot of people.

I should be "¿Cómo está usted?" - And it means "How are you?" with the formal "you".

You know, Spanish has 2 type of "you". One is informal (Tú) and the other one is formal (Usted).
